General Information

Carlton Family Medical offers standard 15 minute or longer 30 minute appointments.

All patients that want to see a doctor require an allocated appointment time. Appointments can be booked online here.

Our practice nurse is available for consultations both booked and emergency and she can ensure a doctor will be available to attend if the condition is urgent.

GP Mental Health Care Plans and referrals to counselling
All our doctors are experienced in managing mental health care problems (includes depression, anxiety, grief, psychosis, relationship problems, etc). Our doctors can develop a GP Mental Health Care Plan (GPMHCP) with you that entitles you to Medicare assistance to help pay for counselling. A GPMHCP entitles a person to six sessions with a psychologist. Another four sessions are available per calendar year after a review of the GPMHCP is undertaken with your GP. The fees for GPMHCP are determined by each doctor and documented in the fee documents at reception.

Home Visits
Home visits are available for regular patients of the service who live in close proximity to the practice and who are too ill to attend the practice at the time of consultation. All visits must be booked in advance. Please phone and speak with reception to determine if this is available and the fees.

Cancelling Appointments
If you are unable to attend for a scheduled appointment, please contact us to cancel or change your appointment time. A fee of $92.00 will be charged for failure to attend or cancellation with less than 4 hours notice. Please note this charge is not Medicare rebatable.

Email Access/ Repeat prescriptions
You can email the practice on info@carltonfamilymedical.com.au we are unable to accept appointment bookings or cancellations by email and ask that you utilise HotDoc for these services.
***Email is not suitable for urgent medical concerns. Please phone reception if your enquiry is urgent or needs same day reply.
Please note there is a fee for doctor services provided via email.
Reprinting documents such as lost referral letters, lost scripts, and copies of specialist letters, misplaced pathology or radiology requests: Fee $33.
Repeat scripts for regular medications, provided the doctor has approved prescriptions without consultation and has seen you within the last 3-12 months (time varies according to the doctor): Fee $33
The doctors at Carlton Family Medical generally will not provide consultations or give advice or issue referrals or prescriptions via email for new conditions or referrals. If this is requested and the doctor is prepared to consult via email the fee for these services start at $95.
All fees are payable via credit card or in person at Reception and there is no Medicare rebate available for any of these services.

NURSE CHILD IMMUNISATION CLINIC
Our Practice nurses are able to see all registered children for routine childhood immunisations. These consultations are bulk-billed. If you book to see your doctor for childhood immunisations there will a private fee charged.
